VitalCare Family Practice is seeking a Board-Certified Family Medicine Physician to join our growing team.  Our office is located in a busy and growing suburban area of Chesterfield, Virginia. 

VitalCare is a new privately owned and operated practice with one physician and two advanced practice clinicians.  We are looking to add a family medicine physician that is interested in practicing high-quality low-cost medical care and helping our practice grow.  We believe that providing quality healthcare can be a very rewarding experience in the right environment. 

We are committed to providing well trained staff, a beautiful modern well cared for facility, and a happy environment to work in. We currently have an in-house lab run by LabCorp, on-site allergy testing, and an on-site behavioral health collaboration.   We use eClinical Works EMR and participate with Virginia Care Partners Clinical Integration Network and ACO.

The MA Care Manager/Care Coordinator’s primary responsibility is to administer care management and coordination activities for the patients of the primary care practice. This will include developing and monitoring care management and care coordination processes and support primary clinical teams with these efforts. It will also include identifying the high acuity patient population and working more directly to ensure care management, as well as care coordination for this patient population. The position may involve some patient triage. The MA care Manager – Care Coordinator behaves in a professional manner, and consistently demonstrates and promotes the values (core values of organization listed). The Care Manager – Care Coordinator will work with the Practice Manager and lead physician of the practice to develop this position to best serve the needs of the patient panel and the primary care teams.

RESPONSIBILITIES:

CARE MANAGEMENT

  • Work with all clinical teams as a resource on care management of all patients of the practice, this would include the following:
  • Pre-visit planning workflow to ensure care completion prior to visit whenever possible
  • After visit summary review with patients whenever appropriate
  • Involving the patients in activities to improve their health (patient engagement);
  • Educating the patient about self-management tasks they can undertake to gain greater control of their health status;
  • Works with physician leadership to define quality measure/outcome reporting process
  • Works with physician leadership to develop protocols and point of care follow ups
  • Serves as a resource to clinical staff and providers to establish quality goals using reports
  • Maintains strict confidentiality; follow HIPPA regulations
  • Treats staff, physicians, NPs/PAs, visitors, patients and families with dignity and respect
  • Participates in professional development activities
  • Performs other related work as required
  • Actively manages assigned panel of chronic care patients (high acuity):
  • Develop relationship with patient as an integral member of team
  • Provide follow-up contact with patient as indicated to ensure compliance with recommendations –medications, lab/x-ray, specialist visits, PCP visits, dieticians, CDE, etc.
  • Manage many aspects of the patient’s care: referrals to specialists, hospitalizations, ER visits, ancillary testing, and other enabling services
  • Responsible for being available to provide telephone advice per protocol, handle urgent calls and emergent calls
  • Anticipate the needs of this patient population, seeing that necessary documentation and pre-visit planning is completed or requested before patient visit;
  • Responsible for working with patient and patient’s care team to coordinate change readiness, needs assessment and develop an individualized treatment care plan
  • Assist patients in setting SMART goals for self –management, teaching them how to do self management tasks and report abnormal findings to their physician team;
  • Collaborates with the patient, physician, and other care team members in assessing the patient’s progress toward individual health care goals;
  • Assess barriers when patient has not met treatments goals, is not following treatment plan of care, orhas not kept important appointments;
  • Oversees the development, procurement, and adoption of patient self-management educational resources used by the primary clinical teams;

Virginia Ear, Nose & Throat, with the Virginia ENT Surgery Center, is recruiting for an experienced, energetic senior manager to assume the role of Chief Executive Officer. Virginia ENT has 4 clinical offices, a 2-room AAAHC accredited surgery center and 2 IAC accredited Mini-CAT CT scanners. The practice has 12 physicians, a nurse practitioner, 11 audiologists and physical therapists as providers. The departments in the practice include ENT, audiology, allergy and balance. Allscripts is the practice management/EMR platform for the practice and HST is utilized in the surgery center.

The practice provides comprehensive ENT and related services to patients of all ages. The physicians are very forward thinking and they are interested in finding a qualified administrative leader to work with them and the senior managers in successfully growing and developing the practice in the years to come.

Responsibilities of the position include, but are not limited to:

  • Fiscal management of the PC, the Surgery Center and the LLC.
  • Working collaboratively with the Board of Directors on all Governance issues related to the 3 organizational units.
  • Monitoring and negotiating insurance contracts in relation to the practice and the surgery center
  • With the Director of Operations, managing the IT/Data Network including a VoIP phone system and onsite virtual servers.

Qualified candidates will have a strong financial background and data analysis skills. A strong working knowledge of business office functions and a knowledge of Quickbooks are highly desirable. An understanding of IT networks, excellent critical thinking skills, experience with strategic planning and operations are also desirable.

Richmond Nephrology Associates is looking for two PAs or NPs to help with night call. This position would pay well and be 7 nights on 7 nights off, so only work 26 weeks per year. The ideal candidate would have inpatient nephrology or ICU experience.

Essential Functions

  • Patient Care during clinic
  • Room patients for providers in the clinic using EMR
  • Obtain accurate vitals, current medications, history, review of systems, reason for visit, and other items as per clinic workflow expectations
  • Perform injections such as Pneumonia, Flu, Xolair, TB Tests, etc.
  • Perform Spirometries and 6-minute walks
  • Administer oxygen and nebulizer therapy when needed
  • Instruct patients on the proper use of inhalers
  • Perform flu and strep testing
  • Complete provider orders, to include:

– Procedures such as sleep studies, PFT tests, CT scans, PET scans, etc.

– Prescriptions through eRX

– Labs

– Referrals to other providers

– DME orders for oxygen, nebulizers, fax orders for CPAP machines and supplies, etc.

– Misc. orders, such as Pulmonary Rehab

– Follow up appointments

  • Accurately document patient activity, phone calls, etc. in EMR
  • Daily duties outside of clinic include:

– Preload and scan patient charts into EMR

– Prep charts prior to office visit by ensuring all recent notes and results are in the chart

– Return patient messages within the deadline established by Administration:

o Rx refills

o Miscellaneous patient questions

– Obtain prior authorizations for procedures and medications

– Respond to healthcare providers’ questions and requests for information (received via fax, EMR message, voice mail, etc.) such as DME companies for oxygen set up, physician offices needing patient notes, pharmacies for RX refills, etc.

– Other items as requested by providers

  • Adhere to NVPCCA standards of excellent customer service to patients, caregivers, providers, coworkers, and those outside of NVPCCA.
  • Adhere to HIPAA policies and procedures in order to protect patient privacy and security
  • Perform other duties as required by the providers, supervisor or member of Administration, for example – cross trained to perform patient check in and / or patient check out
